Carers have a duty of care to report any concerns regarding their client’s well-being to their supervisor. If a carer witnesses poor practice, abusive or exploitative practice they should use this form to document their concerns and send it to the appropriate office. If your concerns regard an office member, you should send this form to Linda Beckett (General Manger) at the Omagh office. All concerns will be treated confidently. If you wish to remain anonymous you do not have to include your name- however we would stress that anonymous concerns are more difficult to resolve.
